Funded provider coordination

HopeMatch Networks

Support coordination between the organizations you fund.

Providers may already know and trust one another. Networks gives them a shared way to manage referrals and follow through around an existing coalition or a committed group.

Your foundation can fund the Network without managing individual referrals. A designated lead oversees participation, and providers remain responsible for care.

Make warmer handoffs.

Providers send referrals with relevant context and permitted intake information instead of leaving participants to carry everything.

Keep coordination visible.

Questions, responses, and status updates stay connected to the referral so teams can see what needs attention.

Understand the result.

Providers record whether services were received, why a request could not be fulfilled, and whether another option is needed.

Your support can cover technology, onboarding, training, and agreed coordination. Approved nonprofit members participate under the funded agreement.

Your contribution may include more than funding: a trusted identity, provider relationships, a resource guide, or the ability to bring organizations together.

Providers retain their missions, professional judgment, and service decisions. Existing applications and internal systems can remain, with integrations evaluated separately.

A digital resource experience can complement a trusted printed guide rather than requiring you to eliminate it.

Funding a deployment does not provide unrestricted access to participant information. Consent, permissions, and agreements still govern access.
